Look at the guys tattoo it is a tape worm. I think red women is liver and fighting with other part of body because of the tape worm. The shipwreck is the food the tapeworm was in.

and either way, the joke is that the drama of the body is unexpectedly literal and her death is thus like a good show being cancelled

The recurring doctor character was listening to the blue-haired person's stomach and there were people inside. The people are fighting, but it's a fun, dramatic, miniseries-style fighting, so even though the blue-haired character is having stomach pain, the doctor doesn't want to cure them just yet.

Since they were too busy observing the drama in the blue-haired character's stomach, the fighting which was causing the pain progressed, and killed the blue-haired character. The doctor is equating this with having your favorite miniseries cancelled.

...It's kind of obscure, but I enjoyed it. All my favorite shows get cancelled, too.
