One thing you have to realize is that a LOT of Americans HAVE very good health care and good vacation time. I do. I don't have to worry about either of those things.
In fact, there's a lot about the US healthcare system that is actually better than other countries. If I want (not need) an MRI, I can literally get one today.
Healthcare is more widely distributed in almost every other country, and I really really wish that we could get our shit together and separate healthcare from employment, I am voting, and donating, and I even went to a few protests about it.
I have vacation time. Many of my coworkers don't even use up all their vacation time. They don't want to, cause they have an unhealthy relationship with work, but it is available to them.
I live in a state that specifically passed laws (that I supported, and voted for) that specifically say we support gender affirming care and abortion rights, even for people traveling from states that don't have those protections.
The US isn't one place, every state has it's own laws that protect people in different ways. Many of the cities also have their own protections as well.
I can move somewhere to get the exact same thing I have now, or I can stay here and keep pushing to help get EVERYONE else in this country the same quality of life that I have. And that's what I'm going to do.
