It doesn't have to be too fancy. It could just be a simple Bootstrap 5.0 thing. Just needs to function for now.

It's useful data. You can see which domains have been checked by others as not-spam and which have been endorsed by other instances. How you use this data is up to each instance, but once it grows large enough it could be used for spam detection, or for whitelisting.

So the determination is whether the entire instance is spam or not?

For guaranteess, yes. Either being dedicated to spam, or overrun by spam accounts etc
