We got nearly wiped by a mindflayer like that once.
I was playing a one off campaign with a lvl 1 character. I forgot how the mechanism was, but essentially the enemy rolled too perfect rolls in a row, and casted fireball, the only thing left of my character was a scorch mark on the ground.
Playing carrion crown and my dm rolled only four successful times that session, three of them were against me, using my only offensive(ish) spell i prepped and nullifying every hex i used. Against peasants with no bonus to will saves. The other was on an npc.
