I think Luffy's personal attitude is that the countries and their leaders aren't the problem, the celestial dragons are. His only real priority if he even has one other than being king of the pirates is fucking them. Country leaders can be good or bad to him, and he's quite happy to swap a bad one for a good one if he can. If anything this is slowly building him a base of revolutionary support that will eventually all be turned on the real oppressors - the nobles. The "countries" are more like small towns/villages that the anarchists visit, do mutual aid with, help, and then move on to somewhere else leaving behind the seeds of revolution. It's a very anarchist-ey story of building his allies up over time through going to places and doing good deeds.
But yeah he would 100% support anything that seems like it would be better than what currently exists. Also he wouldn't give a shit whether people dislike him for doing it too. He's a pirate, not a hero, as he says in his own words.
Also people keep forgetting the fact that the World Govt. would never allow islands like Arabasta to be ruled by a leftist leadership. Much of the post arc of arabasta was showcasing how Luffy's success in liberating the country is limited by the hegemony imposed by the world govt.
Tldr: it would be even more irresponsible of luffy to get rid of the monarchy since it would put those islands on the hit list of the marines and cipher pol. More of this can be ascribed to the revolutionary army rather than luffy and co.

The latest chapters demonstrate that the world govt. is willing to nuke entire islands just to show off their power.Also all the people luffy defeats are people who choose to preserve the current system to their own benefit. Doflamingo (and Kaido) funded wars, crocodile attempted to create a military dictatorship, Enel represents the WG's view of the world (much of skypia is just a pictaresque view of the entire story).
