I have worked in several hotels. None of them charged for visitors. I’m calling bullshit on this one.

If this were in Central or South America, or Asia it's believable. I remember checking in one time and there was a sign on the wall saying something like "female guests will incur an extra charge".

That happens because we are still pretty conservative in Asia and large parts still don't have as open a culture of relationships as the west. Young couples generally book these rooms (or love hotels, as they are called in Japan) to have sex. Obviously that's looked down on. This is a great way to make money on the societal restriction lol.

It looks like the back door, but it's actually more like the front door once you get in. I prefer it because it's a little different than the front without the risk of the back door (sometimes they really don't like it).

In my experience, cheaper hotels charge per person and don't allow visitors at all; more expensive ones charge per room (though with a limit for visitors or extra guests).

IMO In reality this can only apply if you can't bypass the reception desk (e.g. a motel).

Additionally some states and countries require that all guests are registered and have shown some id, so if you wanna go up you've got to check in.
