It looks like it's supposed to be a single lung. It looks identical to one from frame 2
I guess you’re right. Bro draws lungs like hearts. But it still begs the question. Why is the doctor conjuring the lung out of thin air.

It’s a little opaque but he’s doing a black power salute to the black dude because he has black lung.
Oh I see, thanks. I thought the doctor took his hair for some reason and the black lung spread to his skin.
Because he has the black lung he thinks he’s full on black now.
This should have been 4 panels
But it's called "huge cartoons". It has to have six at least! /J
