When tapping on the image of a post from the post feed screen, user will frequently get a placeholder image rather than the image in question.
Not the dev, but lemmy.world's uptime is very bad. I assume your broken images are from lemmy.world, since your user is located on that server. If the source is not hosting content correctly, there isn't much that you can do client-side.

This is probably the issue. I still think there should be something that tells the user that there is connectivity or throughput issues (progress bar or friendlier errors).

I've seen that. What's weird is the app appears to load the right image, then once it's done loading, replaces it with that ^
Sometimes reloading the image works.
